  • Note:- Please select and fill the mandatory fields marked as asterisk (*)
  • Document Name: - User has to enter the name of the document which he is going to submit for Ex. Staff Approval Letter (Mandatory).
  • Document File:- User has to select the scan file of the document whish he wants to upload (Mandatory).
  • Upload Document: - User can upload the document by clicking on the upload document button
  • Cancel: - Clicking on the cancel button will cancel the uploading process.
